The pre-examination assessment.

I understand and agree that:

1. The examinee(s) will not be permitted to take the polygraph examination if they have consumed alcohol, abused controlled substances, or taken recreational or non-prescribed drugs in the 24 hours before the test.
2. If an examinee is pregnant at the time of the examination, or has given birth in the four weeks before it, a doctor’s note is required.
3. Neither I nor the examinee(s) have a heart condition requiring blood pressure medication or a pacemaker. If any of those apply, I will consult a medical doctor before accepting these terms.
4. The person booking, and every examinee, must be at least 18 years old.
5. We ask you to arrive as your normal self, so you should not withdraw from any prescribed medication for the purpose of the test.

I understand and agree that:

1. I have shown these terms and conditions to the examinee in full, and the examinee has agreed to be examined.
2. I must provide government identification — a photo ID card, passport or driving licence — on arrival, for verification.
3. Abuse, aggression or threatening behaviour towards any employee or affiliate of Lie Detector Test UK will not be tolerated. It will end the examination, all fees paid will be retained and the option to rebook will be withdrawn.
4. If any employee or affiliate is exposed to criminal activity, underhanded behaviour or an attempt at bribery, they may end the examination. All fees paid will be retained and the option to rebook will be withdrawn.
5. Any attempted countermeasures will result in one warning. A further attempt will end the examination, all fees paid will be retained and the option to rebook will be withdrawn.
6. I must arrive at the venue at least 15 minutes before the allocated time. Unreasonable lateness will cancel the examination, all fees paid will be retained and the option to rebook will be withdrawn.
7. The date offered may change because of the examiner or examinee’s health, the office being unavailable, equipment failure, or another unforeseen reason. If it does, we will always carry out the test within 10 working days of the date you originally chose.
8. Only closed-ended, issue-specific questions are valid within the examination. Questions about beliefs, opinions or intentions cannot be asked.
9. You are asked up to eight questions during the examination, of which two or three are closed-ended and specific to the case.
10. The lie detector test uses a polygraph instrument to detect physiological signs of deception, alongside the work of a forensic psychologist.
11. If there is any suspicion of domestic violence, the examiner will report it to the relevant authorities, without exception.
12. No third parties may be present during the examination itself. They may be present during the pre-test interview.
13. If I am providing the venue, the room must be clean, private and uncluttered, with two chairs, a table and a power socket, and the examinee must be able to be left alone with the examiner for up to two hours without interruption. We may refuse to proceed if these conditions cannot be met, and any fees paid will be retained.
14. A full written report is provided by email within 24 hours of the test, or within 48 hours if sent by tracked mail, whichever the client asks the examiner for on the day. In certain circumstances the examiner will give the result verbally on the day, to the person who made the booking. Where the charts need cross-examining by a second examiner first, no verbal result is given.
15. This examination does not relate to any illegal or criminal matter. If that later turns out not to be true, we may cancel the booking with no refund.

I confirm that I accept, and will adhere to, all of the following:

1. Once we have received the full and final payment we will disclose the location of your examination. The examiner will formulate the test with you at the venue, immediately before the test takes place.
2. By signing this agreement you confirm that you are the authorised cardholder and have the authority to make an electronic card payment. Processing this or any transaction without that authority constitutes fraud, and Lie Detector Test UK is not liable for fraudulent activity.
3. When making a booking you agree to two payments: the initial non-refundable booking fee of £125, and the remaining balance for your polygraph test.
4. A booking made with less than 48 hours’ notice is still charged in full, and every term here still applies.
5. We take the initial non-refundable fee of £125 at the time of booking. The remaining balance must be paid in full no later than 48 hours before the agreed test date, and your completed pre-examination assessment must be submitted before then.
6. Your pre-examination assessment must be completed within 24 hours of the non-refundable booking fee being taken. If it is not, we may cancel the appointment.
7. Any additional supporting documentation we ask for must reach us at least 48 hours before the examination. We will contact you before, and at, that deadline. Failure to provide it will cancel the booking, all fees paid will be retained and the option to rebook will be withdrawn.
8. To move the date of your booking you must give us at least 48 hours’ notice, and the new date must be within 30 days of the original.
9. Your examination date must be agreed within 30 days of your original booking date. If it is not, we may close the booking and the non-refundable fee stays non-refundable; anything you book after that time may need to be a new booking.
10. On a cancellation the non-refundable fee can be transferred to another appointment with no loss of payment, provided the cancellation is at least 48 hours before the appointment. Cancellation requests go to team@liedetectortest.uk.
11. In unforeseen circumstances where we agree the 48-hour period has been met, you may cancel the scheduled examination and will receive a refund of the balance, less the non-refundable fee.
12. Failure to adhere to any term of this agreement will cancel your examination, all fees paid will be retained and the option to rebook will be withdrawn.
13. Once the test has been conducted and the results provided, no refund is due, whatever the outcome — No Deception Indicated, Deception Indicated or Inconclusive. The results given are final.
14. Any payment made to us is final. These terms supersede the terms of your bank, banking institution or creditor, including but not limited to Mastercard, Visa, PayPal and Payl8r.

I confirm that I accept, and will adhere to, all of the following:

1. All data, charts, documents and video recordings remain the property of the examiner, and are handled in accordance with our duties under data protection law.
2. If you state on the consent form at the outset that you want your test data removed once our work for you is finished, then on production of the report we delete all data obtained, with the exception of the polygraph results report, which we retain for one year under our American Polygraph Association terms of membership. If you do not state that, all video, voice recordings and charts we hold are deleted automatically 24 hours after the report is produced. If you state on the consent form that the video and voice recording are to be used as evidence in a court or tribunal, or for any other legal purpose, we retain that data for a maximum of one year. During that time the recording remains the property of Lie Detector Test UK; if you asked during the pre-test interview for the video to be kept, it can be viewed with an examiner at a controlled office on an agreed date, subject to covering our costs, or where a legal body requires it. The video and test data need interpreting by our examiner and would not be released other than on those terms. You must tell your examiner at the time of the pre-examination interview, or such recordings are deleted within 24 hours.
3. We reserve the right to share your personal data and test data with the police, or another relevant body where necessary, if the results indicate deception related to allegations of sexual abuse or domestic violence.

I confirm that I accept, and will adhere to, all of the following:

1. We are not liable for any failure to perform, delay in performance, or any of our obligations under these or any other implied terms, caused by events outside our control.
2. An event outside our control means any act or event beyond our reasonable control, including but not limited to: lockouts or other industrial action by a third party, civil commotion, riot, invasion, terrorist attack or the threat of one, war whether declared or not, fire or explosion, or an act of God such as storm, flood, earthquake, subsidence, epidemic or other natural disaster. If such an event affects the performance of our obligations we will contact you as soon as possible to tell you, and our obligations under these terms are suspended for its duration.
3. If an event outside our control occurs we will confirm with you whether the scheduled appointment will be met, or an alternative arranged. No refund is made because of an event outside our control.
4. In unforeseen circumstances that we agree and accept, and with at least 48 hours’ notice, you may reschedule the polygraph examination within 14 days of the original date, and we will hold your original non-refundable fee against the new booking.

4. Events outside our control.
5. I have also reviewed and accept the Service Terms & Conditions.
6. I wish to proceed with the polygraph examination. I am authorised to agree to these terms in their entirety, and to make full authorised payment.
